DESCRIBE THE INCIDENT: I was turning the patient in room 343 per standing orders to prevent bedsores. I heard a commotion from the next room over, and found Timothy Delgado attempting to stand up. I tried to reassure and help orient him. When I reached out to keep him from falling over, he did something to my right arm and the next thing I knew I was on the ground and in severe pain, about an 8/10. I distinctly remember thinking that I had broken my arm, and that at least I could feel my legs. I think I lost consciousness, or the head trauma took out my memories. When I came to I was completely intact. There was blood all over the back of my skull, but I wasn't bleeding. My left shoulder and head were inside of a cabinet door I had smashed through. When I tried to extricate myself, I accidentally smashed through the remains of the cabinet. It was like they were made of Styrofoam. Timothy Delgado was clinging to me, and I was unable to unclamp his hands. He was suffering from disordered consciousness. I attempted to communicate with him but he was largely nonresponsive. His mother attempted to do the same, but our combined efforts failed to dislodge him. After several minutes, he lost consciousness altogether, at which point his grip slackened and we were able to move him to an improved silo pending guiding.
